Enroll Now :: Online Application Form
All students must submit a Birth Certificate and Immunization Record to complete their application. Student Applicants for 10th and 11th grade, will also need to submit an unofficial transcript from their current high school. Items may be:

- Sent via mail Attention: Records, 1251 E. Southern Ave, Tempe, AZ 85282
Incomplete applications will not be processed.
Please note, completion of the application does not guarantee enrollment into Tempe Preparatory Academies. Most grades have a waiting list and space availability must be reviewed before we can offer enrollment to applicants.
**Once the online application has been started, you cannot save and come back at a later time to complete the application. Please be sure all parties filling out the application are present and that the application can be completed in one sitting. Thank you.**
